You dont call to another ISP/Provider over some peeringpoint, in fact you
dont call anywhere at all that has an IP-address, you call a phone-number,
and it goes over PSTN.

Thats this thing you pay for. Access to PSTN. A Phonenumber. If you want
to call someone who has an IP-address, install NetMeeting, or one of 10+
free applications.
