I recently came by the following announcement in the online version of
the Newsletter of Systematic Ichthyology:

I did write to that e-mail address about 3-4 weeks ago, but have received
no answer so far. I am definitely interested in Mayden's paper so I would
be grateful if anyone could tell me how I may get a copy of it. Thanks
